Cara,
Get the County Seat (Courthouse town--generally listed in the index of road
map books by state) and call or write to the Department of Vital Records.
They should have the original Death Certificate and should be able to
provide you with a certified copy for a reasonable fee.

Actually as small as it is, St George is the BIG city for that part of
Utah. My in laws live in Milford and drive the hundred miles to St.
George when they go to "town". We're talking RURAL.
