Application For Widows To Be Placed On Pension Roll After Death Of
Husband, Under Section 8, Pension Laws Of September 23rd 1919

Pike County
To The Probate Judge of Pike County;
Mary Fortune, the widow of the late Elijah Fortune, who died on Feb.
11th, 1920 and who was at the time of his death, on the pension roll of
Pike County as Number 7338, under section 4 of Act approved September
23rd 1919, respectfully petition that my name be placed on the pension
roll of Pike County as such widow, and I further affirm that I have not
remarried since the death of my husband. I further certify that I
married him prior to June 1st, 1900; at time of his death, I was not
divorced from him, and was not voluntarily living apart from him, during
the period of one year, prior to his death, and I do not own interest in
real estate, or personal property to the amount of $5,000, and that my
income does not exceed $1,200.00 per annum.
