Here's what Ancestry.com
(http://www.ancestry.com/ssdi/ss-5.asp) states:
The application form (SS-5) contains the following
information:
Full name
Full name at birth (including maiden name)
Present mailing address
Age at last birthday
Date of birth
Place of birth (City, county, state)
Father's full name "regardless of whether living or
dead"
Mother's full name, including maiden name, "regardless
of whether living or dead"
Sex and race
Ever applied for SS number/Railroad Retirement before?
Yes/No
Current employer's name and address
Date signed
Applicant's signature
<end quote>
I've received the SS-5 for most of my ancestors. The
time is reasonable, usually within a couple of months.
Worth noting (I imagine this was common), on two
SS-5s I received, "mother's maiden name" was listed as
UNK.
Value can be obtained far beyond the mere facts with
this document. When I got my fathers SS-5, I read it
to my mother. When I read where he was employed at
the time, she exclaimed that SHE was working there at
the time, they met there and lunch dates were part of
their courting. My father's future brother-in-law
also worked there, and they often lunched together at
yet another ancestor's restaurant just around the
corner from their workplace. This was all terrific
family history that I would have missed out on had I
opted to save the $27.00 SSA fee.
Another personal note: my step-father's SS-5 showed
him employed at Chicago's Wrigley Field (Cubs) at 13
years old. No doubt an absolute dream job for a
baseball fan till the day he died. Family tradition
had him working there as a waterboy, but now I have
the documentation to prove it!
Happy Searching,
Tom Mueller
