I'm hoping someone on the list can help me as I'd like to mail my BCG preliminary application for CG this week. I hate to admit it, but I've been dallying with this for over three years. I'm much better prepared now however than I was in 2002.

I need assistance in completing the section "I have paid the Preliminary Application fee ($50.00*):
________ by enclosing herewith my check or money order, payable through a U.S. bank; OR
________ by pre-purchase of an application package, as verified by____________.

After purchasing the BCG Genealogical Standards Manual at Barnes & Noble in Aug 2002, I ordered The BCG Application Guide online in October 2002 using my MasterCard.

My quandary is in how to complete the part:

"as verified by__________."

I have no receipt from BCG, only a sticky note attached to the On Board that came with the Application Guide that states:
Betty Lou, Your subscription will begin with the Jan 03 issue. Lynn McM. Ex. Dir.

Your assistance is appreciated so that I can mail my app before the two weeks mentioned by Elizabeth expire.
