My great-uncle Floyd was nicknamed Doc. In fact I never knew his name was
Floyd until I started filling in the family charts when I started doing
genealogy. I never learned where he got that nickname.
Then there was my husband's great-aunt Blanche who was named Doc since she
was cute as a bunny ("What's up, doc?").
Oh, the stuff of family stories!

Are there specific reasons someone might be called "Doctor," when the
person is not a medical doctor, vet, or salesman of remedies? The
family in question actually thought he was a medical doctor but it
appears to be merely a nickname or possibly a term of respect of some
sort.
