I agree that the mothers surname is Mulvehill.
Apparently this document is a transcription made in 1916 of the Birth
Registration entry . You can get the birth Registration films from
the FHL. The 1886-1887 Index of Births film for 1886 is 0101061.
However "Births for 4th quarter of 1881 through 1900 1st quarter were not
available for filming" as of 1961. However your transcript indicates
that they do exist.
