A possibility I haven't seen mentioned is that she was actually Sarah Susan(nah) or Susan(nah) Sarah. I have seen several situations where someone was called by her middle name as a child, then by her first name as an adult (or vice versa).

Of course, much depends upon time period, location, ethnicity, etc.
