How To Obtain Arkansas Vital Records:
Arkansas began officially recording births and deaths 15 Feb 1914, though compliance
was lax until 1921.
For Birth, Death, Marriage and Divorce certificates, request the proper form from:
Arkansas Department of Health
Vital Records Division
4815 West Markham Street Slot 44
Little Rock AR 72205.
Birth certificates are $8, Death certificates are $8, Marriages and Divorces are $8,
as of 01 Jan 2000.
Here is a toll free number for the Arkansas Department of Health, Vital Records
Division
1-800-637-9314.
Their web site address is: <vitalrec.com/ar.html> You can go there and print the
forms you need.
A death certificate will tell you when and where born; spouse, or widowed, or
divorced; when, where, and of what died, physician who attended, name and birthplace
of each parent, if informant for death certificate knew this; place buried, the
funeral home which has these records, and the name and address of the informant!
