My mother's Aunt was born in March 1891 and her birth certificate provides
this information:
The District where she was born,
When and where she was born,
Her name and whether she was present or not,
Her sex
Her Father's name and surname
Her Father's rank or profession
Her Father's age and birthplace
When and where her parents were married
The names and ages of other children who were living
The numbers and sex of deceased children
Her mother's name and Maiden surname
Her Mother's age and birthplace
The signature , description and residence of the informant
The names of Witnesses: Medical attendant; Nurse; Others
The name of the District registrar
The date and place of registration
Name added after registration of the birth.
I hope this helps, but I don't know when this type of form was introduced.
