Hi Polly,
The Annual Report of the Secretary of Mines states tha in the N0. 4
District. G.H. Fitches, Mines Inspector Ballarat. Accidents, -- Two
fatal and one serious accident occured. At the North Nuggetty Mine,
Samuel Hender was killed by a fall of rock whilst boring with a
machine. At the Ajax, Daniel Sullivan was boring with a machine when
a large rock fell from the shoulder of the drive, killing him. At the
Llanberris N0.1, George Ling received a badly crushed foot by a fall of rock.

That is all that is reported in that report.
