It seems that I missed this one when I was forwarding complaints. I had
several formal complaints come into me about this time and I guess I didnt
get them all forwarded.

I would like to formally apologize to Mr. Spidell and the Board for my
oversight. I would also like to publicly reprimand myself for my actions.

Please find below Mr. Spidell's formal complaint/grievance.
