As I promised, below are my detailed instructions of how the report was
created.

Within TMG, go into the Custom Report Writer. I selected one of the sample
"Genealogy Report (Register)" reports, copied it and then made the
necessary changes.

On the General Tab,
Publications - I turned everything off, except the surname index
On the Type Tab,
Style - Register format (NEHGR)
Direction - descendants
Blank Line Before each subject
Blank line before each family
Suppress details for living people
# of generations - 5
First reference # - 1
Threshold 0
Include Blank surety
On the Tags Tab,
Tag types - selected, and then I unmarked all and selected only birth,
death, and marriage
BMDB Abbrev - born 1861
Events - Primary variations
Exclusion - show sensitive date
Places - all are checked except include commas when missing
At
On the Misc Tab,
One file per generation
