TO REQUEST MILITARY PERSONNEL RECORDS
YOU NEED TO

1. obtain a copy of Form SF 180
2. complete the form with the fullest amount of info you know about the
military service of the veteran
3. send the completed SF 180 to:

Military Personnel Records
9700 Page Avenue
St. Louis, MO 63132-5100

TO REQUEST MILITARY MEDICAL RECORDS
YOU NEED TO

1. obtain a copy of Form SF 180

2. complete the form with the fullest amount of info you know about the
military service of the veteran

3. send the completed SF 180

4. Medical records are kept at 1 of 58 regional VA centers around the
country or at the national Veterans Administration Records Management Center
in St. Louis, (314) 263-2800.

5. Consult you local telephone directory for the address of your nearest VA
office, or phone toll-free, 1-800-827-1000.

HOW TO OBTAIN FORM SF 180
Send a self-addressed, stamped envelope to:

SF-180 Request
The American War Library
16907 Brighton Avenue
Gardena CA 90247-5420

A copy of Form SF-180 will be mailed to you on the same day your SASE is
received.
