I just did one from 1934 here in Stanislaus (it's on a form that I believe
all counties used and was revised on a regular basis)
this is the information that appeared on it:
Name, address, date of death, place of death, sex, marital status, spouse's
name, birthdate, birthplace, father's name, father's birthplace, mother's
name, mother's birthplace, how long in community, how long in CA,
informant's name and address, burial location, burial date, funeral home,
cause of death, signature of doctor

Of course, if the informaition on the parents was known, the box would have
unknown or a question mark in the box but everything else is usually
complete

All the SF dcs are on fiche. When I transcribed one from 1924 it looked
just like the ones that I see here in Stanislaus. Did the person that
provided the transcription indicate that all the other boxes for information
were blank?
