In my country (Denmark), the most common problem is probably similar as the
describeb

I tend to lean towards the ideas described by John Smith. I think it is
quite essential to keep the name field clean, i.e. just containing the
correct, official name, and then keep the nickname(s) in a field by itself
(as in V2).

Then you could give user a few options on how to use the nickname in each
report. I use Ira's Robert Edward Smith, nickname Bob for Robert, as an
example:
1) In place of the given names: Bob Smith
2) After the official name: Robert Edward Smith (Bob) or Robert Edward Smith
("Bob").
3) Between the given names and the family name: Robert Edward "Bob" Smith
(or (Bob) or ("Bob")).
4) As 3), but with lowest priority to be abbreviated (in this case I like
the "Bob" version best).

As John said, in the Story it should just be Bob Smith.

The problem with people using using another than their first given name as
their common name is handled nicely, I believe, with options 1, 3 and 4.

I also agree with John, that any solution chosen for V3 should be
"compatible" with V2, meaning that it should work in V3 in such a way that
all names coming from a V2 data base can be shown as before ("maximum
backward compatibility" as John called it - I wasn't sure if this was the
correct term).
