I AM FROM CT. I AM LOOKING AT A DEATH CERTIFICATE NOW OF MY GRANDMOTHER
WHO DIED IN NEW HAVEN CT IT HAS HER DATE OF BIRTH, DATE OF DEATH,
HOSPITAL DIED AT, S.S.# HUSBAND'S NAME & PARENT'S NAME ON IT. ALSO HER
LAST ADDRESS.
