This happened in my family. My grandmother's first child was given her last
name, (Paulsen) at her baptism. The father's name was given as Pedersen in
the churchbook. When my grandparents immigrated the child was using my
grandfather's name, Johannesen. This daughter later married my grandfather's
brother and they legally changed his last name from Johannesen to what had been
his middle name, Berg. Sure put a twist on the research.

It was suggested on the List some time ago that a
child would receive the name of the father, even
though the couple was not married. In this case, the
child was named Nielsen (the same as the mother's
name) and not Hansen, had she been named for the
alleged father. There is no evidence that the mother
and the alleged father intended to be married.
