They now have a website and an e-mail address, and anyone contacting them
with a request for copies of documents should visit the website first, as you
can download a search request form *and* a copyright declaration form from
it.

As I discovered from a recent airmailed will request to Lichfield (following
Mike Spencer's wonderful listings), they will not despatch copies until you
have also mailed them (not by e-mail) the signed copyright declaration. By
downloading the declaration form first, and sending it off along with your
copy request, you will eliminate the delay of another roundtrip
correspondence.
