Hi Elizabeth
Since writing my original I've dug out my paternal
grandmother's cert; this is the nearest I get to the
changes; while I can't guarantee the Superintendent
Registrar changed his maind again between 1st June and
13th August 1969 I think it's unlikely!
The format as of 14th August 1969 is tall, not wide
and contains the following:
Entry No.
Registration district
Sub-district
Administrative area
1. Date and place of death
2. Name and surname
3. Sex
4. Maiden surname of woman who has married
5. Date and place of birth
6. Occupation and usual address
7. (a) Name and surname of informant
(b) Qualification
(c) Usual address
8. Cause of death
9. Signature of informant
10. Date of registration
11. Signature of registrar

Indeed! But be careful to make sure you know which
person to believe! The date of birth given on this
death certificate by my grandfather is correct,
whereas the one given on her birth certificate by her
mother is not! The error on the birth cert was only
discovered some time later and they never bothered to
correct it.
