Spittal is actually a place in Pembrokeshire, maybe Common was the name of the dwelling

I have been looking again at a death certificate I got and would appreciate the lists views as to whether my interpretation of the cause of death is likely to be correct.
The death took place 1 Nov 1873 at "Common Spittal" and the cause of death appears to read "Paralysis No Medical Attendance". The death was registered by a relative, who was said to be present, but who lived elsewhere.
My queries are:
1. would it have been normal for death in such circumstances not to have been signed off by a doctor?
2. was "Common Spittal" the poor house?
