Donna - this is not unusual where a child has been baptised soon after birth
at home by a parent or midwife or other person - often, though maybe not
always, because its health is dodgy and it might not survive until a baptism
has been arranged at church.

Later at the church the godparents and parents and priest have come together
to acknowledge the fact that the child has been baptised, and what name it
has been given, and who the godparents are and that they are taking on that
responsibility, and that the child is now part of the Christian community.
The words you ask about mean that the priest, Julius D., has supplied the
ceremonies, i.e. made sure these extra bits have been carried out as they
would have been had the child been baptised in church in the normal way. It
isn't re-baptised as you can only be baptised once and it doesn't have to be
a priest who does it, though there are certain things that have to be done,
and the priest might conditionally baptise if he thinks these haven't been
done, e.g. pouring of water and baptising in the name of God the Father, Son
and Holy Spirit.

Similar entries are sometimes found in Anglican registers - there it often
says 'Received into the Church' or 'Congregation'.
