The message "this file already exists" means "this file" for "this
user/transcriber". So the transcriber must have already uploaded it.
As Allan says it is always extremely helpful (often essential) to supply
information about the actual incident but from the logs I can tell you that
the transcriber successfully upload this file at 06:16 this morning.
