Hi Donna,
You can use Front Page to create the form (guestbook or any other type of
form), but the RootsWeb servers are Unix based and Front Page extensions,
required to make your form *work* only run on a Windows based server.
You can use one of the many free Guestbooks available, including the one at
RootsWeb

Or you can can design your own form, and then set it up to work with
MailMerge. Instructions for that are here:

Using MailMerge, you can have the input sent to you as an email, or posted
to a web page.
