Yes, you can copy code that you find on other folks' guestbook pages --
with the proper modifications.
1) You need to change the wording for the (up to) 3 optional questions to
match the questions that you set up when you registered your guestbook.
2) You'll want to put a title on your "signing" page that matches your site
and guestbook title.
3) You'll need to modify the "<form action=" tag to replace the other
person's guestbook number with your guestbook number.
