Subject: Is Site Not Working?

Yes I am trying to add index to my page and everytime I click Edit Index tab a page comes up saying ERROR wants me to put my USER NAME & PASSWORD page says choose a file. So I was just wondering am I the only one having this problem if so what is wrong then? I have uploaded the pictures and wanting to add index with picture.
