I have accessed the Cemetery survey at my local LDS Family HIstory Center in
Orange, CA. It is on microfiche and part of their permanent collection. They
have the supplemental, too. If I remember correctly, if a patron orders any
item on microfiche (as oppesed to microfilm) it automatically becomes part of
that local FHL perm collection. So not an excellent investment of about
$5.00 as far as I am concerned.

The Original Survey is microfiche #6048749 -- pub 1983 by the Carroll Co Gen
Soc

The supplement is # 6010667 -- Includes an index to the cemeteries in the
original volume, corrections and additions to the original volume, additional
cemeteries, and a surname and maiden name index for both the original cemetery
book and this supplement. Compiled by Myron W. House and the Carroll Co Gen Soc
pub 1989

DISCLAIMER -- I don't have time to do look up these days. Sorry. I haven't
been to it for at lease 6 mos. I do most my work by the internet these days
after the kids go to bed and long after the FHL have closed.
