Try this:
1) Select the Family Group Report "Basic" from the menu.
2) Click the "Clear Keys" button.
3) Change the Report Layout dropdown to "Fill-in Form".
4) Click "Make Report".

You should see a blank Family Group report, with blanks for four children,
each showing a line labeled "Spouse" immediately above a line showing
"Marriage / Place".

If you don't see this: go to the Help Menu and choose "About Genbox Family
History". The program release version is displayed on the right side; it
should say "3.0.16". If not, then your problem is that the upgrade was not
successful and you need to install the appropriate patch files.
