Subject: multistep deletions on markers

Why do multistep deletions on markers occur? Are there any theories/ideas on that? Are they caused by environmental factors, such as solar flares, cosmic rays, famine, etc?
I'm still puzzling over Y-DNA test results that show a multistep deletion on DYS 391, which is usually 10 or 11. I have a value of 7 on that marker. Thanks,
