The "Lump Sum Payment" is a benefit (of $225 as I recall) payable on
the death of the SS beneficiary. Nowadays it's payable only to a
surviving spouse but in the past could be paid to others, presumably
to the person who paid the funeral expenses.

The other ZIP code (LR) is the locality where the beneficiary
received his/her normal SS payments while living.
