Generally there are no family member names unless it is the widow of the
veteran.

By the column headings:
1 House No.
2 Family No.
3 Name of surviving soldiers, sailors, and marines and widows.
4 Rank.
5 Company.
6 Name of regiment or vessel.
7 Date of enlistment.
8 Date of discharge.
9 Length of service.
10 Post-office address.
11 Disability incurred.
12 Remarks.

If you go to http://www.nara.gov/ (main page) then to 'the genealogy
page' and then to 'civil war records' you should find the instructions
and email address Form 80 is for civil war veterans.
Put Form 80 in the subject line and in the message give your postal
address and number of forms you want (limit six per order). You will get
an automated email response asking if you included all the information.
Once you get the forms (in less than a week) you can ask for military
record and pension record but on separate forms. Usually the pension
record gives more genealogical data. I got 20 to 30 pages of pension
application stuff. The widow applied for the widow's pension and for
about 35 years following the death of her husband the government denied
the pension because the veteran could not be found in the files. My best
guess about why is that he fought as a "substitute" under another man's
name.
