Include individuals name, social security #, plus the date and
address at time of death if you know them. You should get your
reply in about 3 weeks. The application form will have the name of
the applicant, the name of applicant's parents, applicant's date of
birth and birthplace. Earlier applications (1937, etc.) will also
include name and address of current employer.
