Steve - sounds like an index card for a Silver War Badge where there is no
service medal entitlement / or / the medals are on a "duplicate" card. Where
you have "Lot" it most probably in fact says "List", its the code for the
SWB roll.

Para 392 xvi of King's Regs 1912 is "no longer physically fit for war
service ..." - covers a multitude of possibilities.
