Sometimes it is mentioned, and sometimes it's not. The same with the
Distinguished Conduct Medal (DCM).

The absence from the MIC is nothing to worry about. Presumably you have
found the announcement in the London Gazette?

There are separate sets of cards for awards of the Military Medal,
Distinguished Conduct Medal and Mentioned in Despatches. These have a
different design to the Medal Index Cards so far available on-line, and
give the London Gazette reference of the award. They also usually give
the soldier's unit, eg 12th Field Company, Royal Engineers.

>I also notice that some cards contain more details than others in that for one relative, there is no mention of his death, for another the cards states "dead" and for the MM it not only says died of wounds but gives the date.

This is another case of "some have it and some don't". Not all MICs for
men who died during the war are annotated to that effect. Some cards also
mention that the soldier was a prisoner, but many don't.

>How very frustrating that they don't all contain the same info when appropriate!

Agreed, but I guess we should be lucky that some of them have useful
annotations, rather than none of them at all!
