In the Oakwood Cemetery VOL I book Macomb, McDonough, IL it shows the following:

Graves, Susan
Bur: 27 Sept. 1904 Age 56
Block 8 Lot 6

No other information is given. I looked up the name Mayfield, and found nothing. Sorry I couldn't find anything else.
