Hi Jim. I think you should contact the Health Dept of Dearborn County. Am not sure of the exact address but it is located on W. High St, Lawrenceburg, In 47025. Postman cannot miss it for it is the only building in that block. If not, it would be the Clerk of the County Court at the same address. Hope this helps. Don Rhodes
