I obtained a copy of my grandfathers birth certificate earlier this year by
picking up the phone and calling Vital Records at the Indiana Department of
Health. Fed Ex delivered it to me in Arizona less than 24 hours later.

Cemetery and address is also listed on this 1912 Indiana Death Certificate.
