We've been looking into the "Women's Christian Temperance Union" quite a bit because we believe our grandmother Emma Rawlings Marshall may have been associated with this. What you're describing sounds like a pledge card, though I've never seen one. The way I understand it, they got men to sign a card pledging not to drink, and that that is the origin of the phrase "taking the pledge" for giving up drinking. Hope this helps.
