I do know what you're talking about and I have used the "Request "For Copies" form myself. In fact, I have some with me here. The form is 2 pages, including full instructions (1 sheet, back and front), but you only fill out the portions that apply to your request. It is Form No. 31768, last updated Oct 2002, so I infer that there has been no price change yet. Birmingham Public Library, Tutwiler Collection, has blank forms for the public, so NYPL probably does also. BUT...you can d/l the form form the LDS website yourself a lot faster (see below).

Copies cost $2.00 each, with a $4.00 minimum. All this is explained and exemplified i the instructions.

NOTE: You may download the form yourself form the www.familysearch.org website and bypass all the locals. The form itself is a little hard to track down within the multiple levels upon levels of pages and menus, so I have copied and pasted the direct URLs for you below.

1-- Go to www.familysearch.org,
2-- Click on the "Library" tab at the top,
3-- Then click n "Services" on the left side,
4-- Then scroll down to the paragraph heading "Ordering Copies by Mail,"
5-- Then click on the link within the sentence that reads, "use the appropriate Request for Photocopies form available on this site."
6-- Then scroll about half-way down the long list and click on "PDF" at the end of the link that reads Requests for Photocopies - Census, Books, Microfilm, or Microfiche (The PDF link at the end of the line goes directly to the form).
7-- Mail your order DIRECTLY to the address at the top of the 1st page:

However, please bear in mine that due to errors and failures-to-find, you might find it cheaper in the long run and more satisfying if you just order the film and do it yourself. Someone wrote that you can get them directly from NY for $12.00 each and that is probably more expensive, but is another option. That would be up to you.

I went to the family center because years ago when I asked this question at another center, they said to do it through them (I think so they get the credit, showing their center is active).
