Marasmus is starvation or malnutrition. Presumably as a result of his injuries
the baby was unable to eat and it was this that killed him rather than scalding
or drowning directly.

Is it possible that the doctor (accepting that it was an accident) recorded the
death in this way to save possible consequences for the sister?
