Adelaide is the capital city in South Australia. If you contact the Registration Office of Birth, Death & Marriages, they'll be able to help you. Apparently a death certificate cost Aus.$29.00.

The BDM site is: http://www.ocba.sa.gov.au/births.htm - it will give you all the information you need.
