need your help please.

I have a death certificate of my G-G-G- Grandfather who died August 18, 1914
in Rapides, LA. HIs name is Curtis Dorsey and was born in 1876 and died at
the age of 38. HIs death certificate is fairly legible but when I get to the
box where it gives the name of the Mortuary or cemetery it simply fades out.
There is a name however that looks like it could be Cortes or Forbes or
??????? The address given for this place is 429 Lee Street.

Would someone please tell me if you know what this could be or if this place
stll exists either in Pineville or Alexandria.
