I think a lot of the questions that are asked can be answered if you read "A MEMBER'S GUIDE TO TEMPLE AND FAMILY HISTORY WORK". However I have found that on page B-3-1 of TEMPLE READY REFERENCE GUIDE it gives a list of Unusual Circumstances for submitting names to the temple.
