When I asked our ward clerk to make a copy of my Membership Record for
me, my individual information appeared at the top of the page. It
contained all my personal information, including membership number and
confirmation date. My husband's and my parent's names and birth dates
and their membership numbers were listed, but not their baptism or
confirmation dates. It did, however, have all the information for our
children.

In an older previous, Family Summary, my husband was listed at the top
with all his information, etc. I guess it depends on the person who
requests their information as to how the printout will be formatted.
So, if both husband and wife request their membership information, you
will get two separate pages.
