Sharon,
That is why I always keep my submission disk until the temple work has
been completed. The Portland temple has had to look at my submission
disk to print out a duplicate name slip when lost. (They lost one of my
slips one time-it ended up in someone else's envelope)

When they do print out duplicate slips, it has the work already completed
printed on it and has the word Duplicate printed on the slip.
