In addition to what Kathie has told you -- are you sure you are going to the
right list's tools page? I ask because the OOPS! message should trigger an
email to the list admin of record with the correct password. If you are not
receiving an email with the correct password perhaps you are attempting to
access a tools page for a list that isn't yours.

PS: Thinking this could account for some of the recent reports of admins
complaining about receiving notices that someone tried to access THEIR tools
pages.
