Someone asked where they could download a form to join.
No form is needed.
All you need do is follow what I've written below.

Just write the info on a sheet of paper and mail with your check.

Remember just a check. Not a good idea to send cash in the mail. We don't
accept pay-pal, credit cards, etc.
I try to think of other things that people may ask, but really, really,
really, joining is as simple as I have tried to explain it. Nothing is
needed beyond your check & the "include your ....." info.
