I received so many questions I thought I would post this.
The info on the request form is as follows:
Name of deceased, date of death, name of hospital or address where death
occurred, city, county, Last known residence of deceased, YOUR signature,
Your address.
If anyone would like one of the forms, I have quite a few and would be
happy to share! A SASE to me, Karen Krugman, P.O. Box 577, Birmingham, MI
48012-0577 and I would be happy to send it.
