I'm going to assume that you did not receive a photocopy of the actual certificate from the Minnesota Dept. of Health, rather you received an abstract. You can get an actual photocopy of the death certificate from the Minnesota Historical Society and you can order it from information beginning at this link:

I am seeking information on the date and place of
burial which was omitted from a NON-CERTIFIED
report of death. I paid my fee for the report.

I sent a letter to the Minnesota Department of
Health, Office of the Registrar and received a
reply requesting an additional fee for another
report.

I explained the whole thing in my letter, but
apparently someone did not read my letter or did
not understand it.

The death report was for DeShene, Alec, died
February 15, 1939. report issued by State of
Minnesata, County of Benton,Gilmanton Township.
His correct spelling of his name is DeShane,
Alex. Born March 1, 1850, LeRoy, NY. HIs
parents name were also misspelled.

I had contacted several Catholic churches with
cemeteries and they had not record.

Could someone please help, so I can complete the
family tree. Thank You
