You don't generally need a form to send for a marriage license
record. Often there is a recording that tells you what info and money are
needed to get a record.
--generally: names of both people, approximate date of wedding, your name
and address.

The office for Henn Co. is downtown Minneapolis in the Hennepin Co. office
bldg. You can make an appoint to check the records on microfilm for 1 (or
more?) hour; see mpls phone book.
