I have a lovely, large-ish document that shows the soldier's name,his spouse, their kids, etc. I think you( they, in the old days)?probably bought it from a printer and filled in by hand the personal details. Mine has yellowed greatly of course to a tannish color. It has in red and blue ink at the top all sorts of frills and flags, curlicues, and bold titles about the grand army. There are many lines below that have been filled in with the soldier, his wife, their children's names, birth dates, wedding dates, and death dates. It is sort of a maxi size of what you might find in the front of a Bible. I? believe there were entry documents, exit documents, and death certificates, but so many are lost. Did you try the National Archives?
I've had no luck whatsoever with finding a similar document for a confederate relative who died during the war. I think there is more success if the soldier survived and drew a pension. That is well documented in most states. Best of luck!!

My great great grandfather was in the 23rd Regiment Infantry Volunteers
Company A. Would he have been issued any kind of certificate as proof
of enrolling or death? If so where does one find copies of what they
looked like. This regiment fought for the north. I have his record of
service card and the file from the National Archives where his wife
petitioned for child support on his death.
